Anoka County Community Social Services and Mental Health Department is establishing a register for current and future vacancies in ongoing Adult Mental Health & Chemical Health case management. This position will provide state and federal mandated and non-mandated services to adult residents having a serious mental illness and/or chemical health issues. This position will work with multiple funding streams such as Rule 25, MH-TCM, and Waivers. Some positions may be contingent on funding.

EXAMPLES OF ESSENTIAL DUTIES (ILLUSTRATIVE ONLY):
Supervises and assists individuals with obtaining basic living needs (food, clothing, shelter); intervenes during crises to provide emergency services and access to related services (financial, psychological, medical, etc.); protects each client’s legal/civil rights/physical and emotional welfare while providing them with the least restrictive living environments commensurate with level of functioning; seeks out appropriate community programs and resources; maintains direct contact with individuals either in the home, in day and residential programs, hospitals, or by phone; seeks out appropriate community based services in such areas as mental health treatment, chemical health treatment, housing, and vocational support/training; authors documents to obtain federal and state and health plan funding reimbursement including MH-TCM, Rule 25 consolidated fund, CADI, grants, MSA, MA and social security entitlements; Completes Rule 25 chemical health assessments, secures appropriate funding, and coordinates treatment planning. Works with County Attorney's office and attends court hearings; completes individual case plans related to involuntary placements, compliance with court ordered services, and guardianship/conservator services; monitors vendors to ensure that clients receive the services contracted and that provider’s billings are accurate; meets compliance with paperwork requirements necessary to maximize state and federal reimbursements; participates in weekly unit meetings, in-service training, conferences and workshops.

REQUIREMENTS:
Knowledge of: Rule 25 and other chemical health assessments including dimensions and levels of treatment, Chemical Health treatment modalities, Illness Management & Recovery: social and economic problems; methods, values, purposes, and principles of social work; state and federal statutes and laws; County policies and procedures; community resources; interviewing techniques; case work methods; mental health concepts and practical application, MA reimbursement procedures. Ability to: Utilize counseling techniques on an individual, family, or group basis; establish and maintain effective working relationships with clients, co-workers, vendors, public officials, and citizens of the community; work with physically challenging individuals; experience in writing and developing Individual Community Support Plans; completing chemical health assessments, reading and interpret technical reports, evaluations, and assessments; work with multiple funding streams and Waivered programs. Skill in: Oral and written communication.
